Instructions

Step 1

Passport valid for at least three months after the end of the dates of your trip to the Czech Republic. It should contain two blank pages to paste in your visa and apply border stamps when you go to the country. Also make a copy of the first page with your photo and information about you.

Step 2

A completed and personally signed questionnaire. You can use English or Czech. It is allowed to fill in both on a computer and manually. Handwriting must be legible and in block letters. Glue one photo of 3, 5 x 4, 5 cm to the application form. The photo should be taken against a light background, without frames, ovals or corners.

Step 3

Confirmation of the purposes of stay in the country. If you are traveling on a tourist visit, please attach printouts or faxes from the booked hotels, which must contain all the details of the reservation. You can show an apartment rental agreement or an invitation voucher from a travel company. Those who travel to the Czech Republic on a private visit must show the original invitation from residents of the country. The invitation is drawn up on an official letterhead and certified by a notary or by the police, if the inviting person is a citizen of Russia. Citizens of the Czech Republic can write an invitation by hand, but you still have to certify it with a notary. You should also attach documents confirming the relationship or clarify the nature of the relationship between the host and the guest.

Step 4

Bank statement confirming sufficient funds for the planned trip. The amount should be calculated based on the fact that for each day of stay per person there should be at least 50 euros in the account. Children require half the amount.

Step 5

Health insurance policy and its copy. It must be valid on the territory of all Schengen countries. The amount of coverage is at least 30 thousand euros.

Step 6

Round-trip tickets. Printouts of reservations for air tickets from the Internet, train or bus tickets are suitable. If you have an original ticket, then you need to hand over a copy to the consulate (you still need to have the original with you when submitting documents).

Step 7

To travel by car, attach the intended travel route and documents for the car: registration certificate and Green Card insurance. You will also need a copy of your driver's license.

Step 8

A photocopy of the pages with personal data and registration from a Russian passport.
