The Czech Republic is an attractive country for both tourism and business. To visit it, citizens of countries that are not part of the Schengen area need to obtain a visa. One of the documents required for this is a questionnaire.
Correct filling of the visa application form is a mandatory item for obtaining a visa to the Czech Republic. This is not difficult to do if you carefully study the requirements for this documents and be careful at the same time. It is worth mentioning that blots or corrections are allowed in this case, but everything must be done carefully. But reporting false information about yourself is prohibited. You can fill out the questionnaire yourself or entrust it to a specialist from the visa center.
What is a visa application form?
An application form or application for a Schengen visa is a form that is issued completely free of charge. It consists of two parts - one is completed by the applicant, and the second is by the representative of the institution that issues the visa, in this case it is the Czech Embassy. The second part is on the right side of the page and is highlighted in color.
The person filling out the questionnaire provides his personal data, the dates of the intended trip, the purpose of the visit, that is, all the information that the responsible person needs in order to make a decision on issuing a visa. In this case, all information must be true and up-to-date, otherwise the visa will be refused.
For answers to visa questions, special columns are highlighted. When filling out, transliteration from Cyrillic to Latin is used. Each letter of the answer should be located in a separate cell.
Completion of the questionnaire by adult citizens
All contact details must be indicated in accordance with the entries in the passport. Those fields that suggest the answer "similar to the previous point", for example, in the column about the surname given at birth for people who did not change it, are left blank.
All dates are specified in the day / month / year format. The addresses are reversed, starting with the street name and ending with the country. In all cases, when no special decoding of the name of the state is required, a unified three-letter code is used.
Phone numbers (of the inviting company, employer or hotel) are submitted along with the country code. It and the mobile operator code are separated from the number by a hyphen.
Features of filling by children
Persons who have not reached the age of majority at the time of submission of documents for a visa also fill out a questionnaire. If necessary, their representative, that is, the guardian, can do this for them.
Employment must be indicated in the "Profession" column. In this case, it will be "student" or "schoolboy". The answer to the next point will be the indication of the educational institution.
Children under 14 years old do not sign the form - their parents do it for them. If the child is 15 or more, then he and one of the parents put the signature.
