Step 1
There is no single application form for vacation, so make it up in any form by hand or fill out a ready-made form from the HR department. Large enterprises often prepare such forms in compliance with the requirements for processing personnel papers to simplify and speed up the office work process. Start filling out the form by specifying the details of the addressee and the sender, located in the upper right corner of the sheet. Write the position, surname, name, patronymic of the head of the company in the "to whom" format. Next, indicate your own position according to the staffing table and the structural unit or department in which you work, surname and initials in the "from whom" format.
Step 2
Place the name of the document "Application" in the center of the sheet. After the request "Please provide me", indicate the type of vacation. This could be:
