You can apply for a passport in Ukraine by contacting the migration service or a travel agency. The cost of services and terms of implementation will vary greatly, but the list of documents will remain unchanged. The necessary basis is an internal passport and identification code.

Instructions
Step 1
For self-registration of a foreign passport, check the days and hours of the inspector's reception at the Visa and Registration Department at the place of registration. At the reception, you must bring an already prepared package of documents, including originals and the required number of copies.
Step 2
Make 2 copies of all completed pages of the passport of a citizen of Ukraine and 1 copy of the identification code. For women who got married and changed their surname, it is necessary to prepare 1 copy of the marriage certificate. If the marriage is dissolved, 1 copy of the divorce certificate. It is important to remember that if the surname has been changed, then the identification code must also be exchanged. The previously issued passport can be returned to the inspector of the migration service, or you can keep it for yourself by paying a small fee.
Step 3
For registration of a passport for a minor child, originals and copies of the following documents are provided:
- child's birth certificate - 1 copy;
- passport, if the child is already 16 years old - 1 copy;
- parents' passports or death certificate - 1 copy each;
- identification codes of parents - 1 copy each;
- certificate of marriage between parents - only the original;
- matte photographs with a format of 3, 5 x 4, 5 cm - 4 pieces;
- permission to travel abroad, issued by the parents at a notary - only the original.
At the inspector's office, parents write an application and fill out a questionnaire, in which they indicate all the necessary data.
Step 4
To paste information about children into the parent's passport, you need to provide 1 copy of the child's birth certificate, make 2 matte photographs of the child with a format of 3, 5 x 4, 5 cm and bring the original parent's passport.
Step 5
After checking the package of documents, the specialist of the migration service will issue receipts for payment of state duties for the services provided. These funds must be deposited to the specified account at the bank's cash desk, and the corresponding receipts must be returned.
Step 6
After confirmation of payment from the inspector, questionnaires and applications are filled out. In addition, you may need a certificate from the Ministry of Internal Affairs about good conduct, a certificate from the military registration and enlistment office, an extract from the address desk or housing office. You may need to take out insurance that requires additional monetary premiums.
Step 7
If you issue a passport at a travel agency, you only need to provide the original of the internal passport and identification code. The rest will be done by the agency staff. At the appointed time, you will need to come to an appointment with the inspector at the OVIR, put your signature on the documents and take a picture.
